If the box is clean - by the cat's standards, not yours, then the problem is most likely a urinary tract infection. Pretty common in male cats, and easily treated by the vet. Male cats can block (and either incur a large vet cost to get them unblocked, or they die) if a UTI isn't treated, so call the vet tomorrow morning and get him in as quickly as you can.
